Skilled nursing care at home is typically recommended for patients who require ongoing medical care and attention but prefer to remain in their own homes instead of a hospital or nursing facility. This can include patients recovering from surgery, those with chronic illnesses, or those who require palliative care.
We offer short and long term care with a wide range of services based on your family’s needs. We have a tiered system of care providers to fit any family budget. Our people-centered approach involves working with patients, their families and the healthcare team, to develop and implement care plans based on our assessment of the patient that address both the physical and emotional needs of patients with the goal of optimal health outcomes and well-being.
Services include but are not limited to the following:
- Monitoring of vital signs
- Medication management including reminders and support in taking required medication as prescribed by the patients care team
- More specialized services such as wound management, elderly/geriatric care, post-discharge and stroke recovery care, Alzheimer’s and other dementia support, as described below
- Respite care for a few hours a day to a few days a week or month, providing family members time to take care of themselves to promote their own self-care
- Personal care services including bathing, use of toilet, grooming, and transfers to support mobility, allowing our patients to remain as autonomous and independent as possible
- Errands and transportation including picking up medications, groceries and shopping, accompaniment and transport to appointments or social gatherings, light housekeeping

We can provide a variety of services tailored to your needs, including elderly care, palliative care, post-surgery support, chronic disease management, care for bedridden patients, or clinically complex cases that require long-term care. Our experienced and qualified nursing professionals develop personalized care plans with our multidisciplinary team, including physicians and physiotherapists, to support physical, emotional, and mental healing and well-being. Home nursing care can be provided short-term, long-term, daily, or weekly.
In addition to ongoing care, we offer IV infusions, lab tests, doctor-on-call, and physiotherapy to provide comprehensive care in the comfort of patients’ homes.
